Claim Instructions


Filing a claim shouldn't have to be difficult. So we've compiled a helpful list of instructions to make the process easier.

  1. Make sure Group Name, Member Name, Member ID and Member contact info are in the email.
  2. Please send EOB and Bill (unless you dont have one for whatever reason.
  3. Make sure Advanced Advocacy and Carrier Consent forms are completed.
